CoPeC - Building Care Pathways for Dementia: An integrated and participatory approach to preserving autonomy

The CoPeC project is dedicated to developing integrated care pathways that enable the timely recognition and slowing of cognitive decline, preserving the autonomy of individuals affected by dementia. Building on the experiences, collaborations, and resources of the long-standing cantonal dementia strategy, CoPeC aims to formalize and effectively communicate tailored and appropriate care pathways for all stages of dementia in Canton Ticino and Moesano (GR). These pathways integrate health promotion, prevention, diagnosis, treatment (including support for family caregivers), and a combination of pharmacological and non-pharmacological therapies, such as psycho-educational, social, and cognitive stimulation.

The project is structured around four key measures: 1) Identification of Entry Points; 2) Identification of Needs and Innovations; 3) Design of Pathways; 4) Communication and Informational Materials.

A cornerstone of CoPeC is the anchoring of a robust network of local stakeholders, such as health professionals, and social and family organizations of patients. The method of collaboration is highly participatory, ensuring a holistic and inclusive approach to defining care pathways, with a strong focus on the individual’s needs.

By leveraging an integrated approach, CoPeC seeks to maximize the potential of existing yet underutilized resources, often limited by visibility, accessibility, or usability. The project brings together diverse skills and perspectives, prioritizing an innovative integrated approach that focuses not solely on the individual but also on the caregiver-person with dementia dyad.
